We provide plain-language tools, structured systems, and educational resources designed specifically for those managing cardiovascular, kidney, and metabolic conditions, whether caring for a loved one or managing their own health.
How Care Partner Guides™ Began
A Note from Our Founder
My grandmother was one of the most important people in my life. Over the years, my role in supporting her health evolved, from check-ins after doctor's appointments to coordinating her care from a distance, and eventually to more than five years of intensive, daily, in-home care as her cardiovascular, kidney, and metabolic conditions progressed.
During those years, I learned to navigate complexity most families never encounter, careful diet modification and monitoring, training to administer life-saving treatments, and more. This demanded organization, tracking, research, asking questions, everything it takes to be a true partner in care. With that partnership and structured approach, she exceeded expectations at every turn.
While working with her healthcare teams, I realized that my level of participation and systematic approach to her care was not common. Most Care Partners were figuring it out alone, without the structured tools or clinical knowledge that could make the journey more manageable.
I knew this work was meant to be shared when people in my life began reaching out, not just for support, but for the kind of practical guidance and organized systems that didn't exist anywhere else. That's when I turned what came naturally to me, informed by years of lived experience and professional training, into a system of resources that could be used by other Care Partners, as well as those managing their own care.
My work sits at the intersection of cardiometabolic science, public-health education, and the hard-won knowledge that comes from navigating CKM care inside a disconnected medical system.
That's how Care Partner Guides™ was born.

Built on Experience and Expertise
Care Partner Guides™ was founded by someone who lived the reality before building the resources.
These credentials didn't come first, clinical-level caregiving did. As my grandmother's conditions progressed, I learned to manage her care in ways most families never encounter: keeping her off dialysis for nearly two years through careful diet modification and monitoring, then training to administer her home hemodialysis treatments when her kidney function eventually declined. The formal certifications came later, validating knowledge I'd already gained through years of lived experience, independent research, and working alongside healthcare teams. Every tool I create reflects that combination: real-world caregiving paired with clinical knowledge, translated into plain language that actually helps.
