Terms & Conditions
Effective Date: August 20, 2026 | Last Updated: August 20, 2026
These Terms and Conditions govern your access to and use of carepartnerguides.com and any related pages, tools, downloads, communications, products, and services made available by Care Partner Guides LLC.
These Terms may be updated at any time as our services, products, and applicable law evolve. The version posted here is the current, effective version.
1. Acceptance of These Terms
By accessing or using the Site, using the Appointment Readiness Tool, downloading a resource, submitting a form, or placing an order, you agree to these Terms and acknowledge the Privacy Policy. If you do not agree, do not use the Site.
If you use the Site for an organization, you represent that you are authorized to accept these Terms for that organization. A separate written agreement, order form, invoice, or institutional license signed by Care Partner Guides may contain additional terms. If there is a conflict, the signed agreement controls for the services or products it covers.
2. About Care Partner Guides
Care Partner Guides LLC ("Care Partner Guides," "CPG," "we," "us," or "our") is a New Jersey health education company. We create plain-language educational resources and organizational tools for care partners and people managing complex chronic medical conditions.
Care Partner Guides is not a health care provider and does not practice medicine, nursing, pharmacy, social work, or any other licensed health profession through the Site.
3. Eligibility and Adult Supervision
You must be at least 18 years old, or the age of legal majority where you live, to make a purchase or enter into a binding agreement through the Site. The Site is not directed to children under 13. A minor may use educational materials only with the involvement and supervision of a parent or legal guardian.
4. Educational Purpose, Not Medical Advice
All Site content, including guides, workbooks, checklists, articles, scores, summaries, examples, and downloads, is provided for general education and organization. It is not medical advice and is not a substitute for diagnosis, treatment, prescribing, monitoring, or advice from a qualified health professional who knows your individual circumstances.
Using the Site does not create a doctor-patient, clinician-patient, therapist-client, fiduciary, or other professional relationship with Care Partner Guides. References to signs, symptoms, medications, care planning, or emergency responses are not exhaustive and may not apply to your situation.
Always ask a qualified health professional about medical questions and before making or changing a treatment, medication, diet, or care plan. Do not disregard professional advice or delay seeking care because of something on the Site.
If you believe someone may be experiencing a medical emergency, call 911 or your local emergency services immediately. The Site and the Appointment Readiness Tool are not emergency services and are not designed for emergency monitoring or triage.
5. Appointment Readiness Tool
The Appointment Readiness Tool ("ART") is an organizational aid intended to help users prepare for health care appointments, identify topics to discuss, and create a personal summary. It is not a clinical assessment, diagnostic instrument, risk calculator, treatment recommendation, emergency alert system, or substitute for a health professional's judgment.
Any Appointment Readiness Score reflects only the responses provided and the tool's organizational framework. It does not measure medical risk, disease severity, prognosis, decision-making capacity, or whether a person is medically ready for an appointment or procedure. No particular score guarantees an outcome.
Review any generated summary for accuracy before using or sharing it. Care Partner Guides does not independently verify user entries and does not automatically transmit information to a clinician, hospital, electronic health record, or emergency service unless the Site expressly states otherwise.
Certain appointment details are designed to remain in the active browser session and not be stored on CPG servers. Information needed to complete a requested function, such as a name or email address used to deliver a summary, may be processed as described in the Privacy Policy. If you download, print, email, save, or add information to a device or third-party calendar, that information is then subject to the security and privacy practices of that device, email provider, calendar provider, or other third party.
If you enter information about another person, you represent that you are authorized to do so and will use and share the resulting materials lawfully and respectfully.
6. Privacy and Electronic Communications
Our Privacy Policy explains how we collect, use, disclose, retain, and protect information. It is incorporated into these Terms by reference.
If you ask us to send a tool output, order update, receipt, or other requested communication, you authorize the related transactional messages. Marketing emails are sent only when permitted, and you may unsubscribe using the link in the message. Unsubscribing from marketing does not prevent necessary transactional communications about a request or order.
Do not use a general contact form or ordinary email for an emergency. Do not send medical records or sensitive health information through a general form unless that form expressly states that it is intended and secured for that purpose.
7. Orders, Prices, and Payment
Product availability, descriptions, prices, taxes, shipping charges, and any product-specific terms are shown on the Site or during checkout. Unless stated otherwise, prices are in U.S. dollars. We may correct an obvious pricing or description error and may reject or cancel an order for suspected fraud, unavailability, legal restrictions, or an error. If we cancel after payment, we will issue an appropriate refund.
Payments may be processed by Stripe or another third-party payment provider. The provider's terms and privacy practices apply to payment processing. Care Partner Guides does not store complete payment-card numbers on its own systems.
An order confirmation acknowledges receipt of your order. It does not guarantee acceptance or availability until the order is processed. Product photographs and colors may vary slightly by screen, printing process, or production run.
8. Digital Products and Downloads
When you purchase or download a digital product, Care Partner Guides grants you a limited, revocable, nonexclusive, nontransferable license to use it for your own personal, household, or authorized internal organizational purpose, subject to any product-specific license shown at purchase.
Unless a separate license permits it, you may not resell, sublicense, publicly post, upload to a shared repository, mass distribute, or provide blank copies of a digital product to others. You may keep reasonable backup copies for your own use, print a working copy, and share your completed personal worksheet or summary with members of your care team.
Digital delivery depends on the email address, device, software, and internet access you provide. If a file is defective, duplicated, or not the product ordered, contact us so we can provide a replacement or appropriate remedy.
9. Physical Products, Shipping, and Delivery
You are responsible for providing a complete and accurate delivery address. Shipping dates are estimates unless we expressly guarantee a date. If an order cannot be shipped within the stated time, we will provide any notice, choice, cancellation right, or refund required by applicable law.
Inspect a delivery promptly. If an item is damaged, incorrect, or missing, contact inquiry@carepartnerguides.com within 7 calendar days after delivery and include the order number and, when helpful, photographs. We will work with you on a replacement, refund, or other appropriate remedy.
10. Returns, Cancellations, and Refunds
The following policy applies unless a product page, checkout screen, signed agreement, or applicable law provides a different or more favorable right:
- Physical products: You may request a return within 14 calendar days after delivery. The item must be unused and in resalable condition. Unless the item was damaged, defective, or incorrect, you are responsible for return shipping, and original shipping charges are not refundable.
- Digital products: Because access is provided electronically, a digital-product sale is final once the download or access link has been delivered, except for a duplicate purchase, defective file, incorrect product, failure of delivery that we cannot correct, or when applicable law requires another remedy.
- Approved refunds: Refunds are returned to the original payment method, generally within 10 business days after approval. Your bank or card issuer may take additional time to post the credit.
To request help, email inquiry@carepartnerguides.com with your order number and a brief explanation. Nothing in this section limits a refund, cancellation, warranty, or consumer right that cannot lawfully be waived.
11. Institutional and Bulk Purchases
Hospitals, health systems, clinics, employers, community organizations, educational programs, and other institutions may require a separate quote, invoice, ordering agreement, business associate agreement, or content license. Retail purchase of a product does not by itself grant enterprise-wide reproduction, distribution, white-label, training, or resale rights. Contact us before making copies or distributing materials across an organization.
12. Intellectual Property and Limited Site License
The Site and its content, including text, graphics, designs, frameworks, scores, product names, logos, downloads, and software, are owned by or licensed to Care Partner Guides and are protected by intellectual property laws. Care Partner Guides and related marks may not be used without written permission.
We grant you a limited, revocable, nonexclusive, nontransferable license to access the Site and use its content for lawful personal, noncommercial purposes. An organization may use purchased materials only within the scope of its written license.
Without prior written permission, you may not reproduce, adapt, translate, distribute, publicly display, sell, scrape, frame, create derivative commercial materials from, remove ownership notices from, reverse engineer, or use Site content to train or improve a commercial machine-learning system. You may share links to public Site pages and may share your own completed worksheets or ART summaries with your care team.
13. Acceptable Use
You may not use the Site to:
- violate a law, regulation, court order, or another person's rights;
- submit information you are not authorized to use or disclose;
- impersonate another person or misrepresent an affiliation;
- introduce malicious code, overload the Site, probe security, bypass access controls, or interfere with another user's access;
- harvest personal information, conduct unauthorized automated scraping, or create a competing database or product; or
- use educational content to provide deceptive, unsafe, or unlicensed clinical services.
14. Third-Party Services, Links, and Trusted Partners
The Site may link to or integrate with third-party websites, payment processors, email services, calendars, embedded media, or other services. Their terms and privacy practices apply to your use of their services. We do not control those services and are not responsible for their availability, content, security, or acts.
Care Partner Guides may identify organizations as Trusted Partners or recommend outside resources based on our editorial judgment and mission alignment. A Trusted Partner remains independent from Care Partner Guides unless we clearly state otherwise. You are responsible for deciding whether a third-party product or service is appropriate for you. Any material sponsorship, referral fee, or other financial relationship will be disclosed when required.
15. Accuracy, Availability, and Results
We work to make the Site useful, accurate, and current, but health information, laws, technology, availability, and product details change. We do not promise that all content is complete, error-free, or suitable for every person or circumstance. We may correct, update, suspend, or discontinue content, features, or products.
The Site may occasionally be unavailable because of maintenance, security issues, provider outages, or events outside our reasonable control. Testimonials, examples, and case descriptions reflect individual experiences or illustrations and do not guarantee that another person will have the same result.
16. Disclaimer of Warranties
To the fullest extent permitted by law, the Site and all free content and tools are provided "as is" and "as available." Care Partner Guides disclaims implied warranties of merchantability, fitness for a particular purpose, title, and noninfringement. We do not warrant that the Site will be uninterrupted, secure, or error-free, or that educational content or organizational tools will produce a particular health, care, financial, or operational result.
Some jurisdictions do not allow certain warranty exclusions. In that case, the exclusions apply only to the extent permitted, and any nonwaivable rights remain in effect.
17. Limitation of Liability
To the fullest extent permitted by law, Care Partner Guides and its owners, employees, contractors, and agents will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, lost data, loss of goodwill, or business interruption, arising from or related to the Site, even if advised that such damages were possible.
To the fullest extent permitted by law, total liability for a claim related to a purchased product or service will not exceed the greater of the amount you paid Care Partner Guides for the product or service giving rise to the claim during the 12 months before the event or $100. This limitation does not apply to liability that cannot lawfully be limited, including rights or remedies that applicable consumer law makes nonwaivable.
18. Indemnification
To the extent permitted by law, you agree to defend, indemnify, and hold Care Partner Guides and its personnel harmless from third-party claims and reasonable costs arising from your unlawful misuse of the Site, material violation of these Terms, or infringement of another person's rights. This does not apply to the extent a claim was caused by Care Partner Guides.
19. Suspension and Termination
We may restrict or suspend access when we reasonably believe a user violated these Terms, created a security risk, harmed another person, or acted unlawfully. Provisions that by their nature should continue, including intellectual property, disclaimers, liability limits, dispute terms, and payment obligations, survive termination.
20. Changes to the Site or These Terms
We may update these Terms as the Site, products, or law changes. The current version will be posted with a new Last Updated date. When required, we will provide additional notice or obtain consent. Updates apply prospectively from their effective date. Continued use after that date means you accept them, except where law requires another form of assent.
21. Governing Law and Disputes
New Jersey law governs these Terms, without regard to conflict-of-law rules. Before filing a claim, you and Care Partner Guides are encouraged to make a good-faith effort to resolve the issue. Unless law requires or permits another location, claims may be brought in state or federal court in New Jersey.
Nothing here deprives a consumer of a mandatory protection or forum right. These Terms do not require arbitration or waive the right to participate in a class action.
22. General Terms
If a provision is unenforceable, it will be enforced to the greatest extent permitted and the rest will remain effective. Failure to enforce a provision is not a waiver. You may not assign these Terms without our written consent. We may assign them in a merger, reorganization, asset sale, or similar transaction, subject to law.
These Terms, the Privacy Policy, product-specific terms, and any signed agreement are the entire agreement about the Site and replace earlier discussions on the same subject. Headings are for convenience only.
23. Accessibility
Care Partner Guides aims to make its website and resources accessible. If you encounter a barrier or need another format, email inquiry@carepartnerguides.com. We will make reasonable efforts to assist.
